We discourage the use of abbreviations, aliases or any form of ambiguity when filling out any of the forms in the request process

Addresses should be in full with details as concise as possible. As much as possible, always provide a zip/postal code if available.

Also ensure to always use your valid email address and phone number when placing a request.
If you are making a request on behalf of a friend, relative or acquaintance, please use the email and phone number of the actual applicant to make processing easier and faster.

Your matriculation number must be entered in full and not abbreviated .
